Title VI of the Civil Rights Act of 1964 prohibits recipients of Federal financial assistance from discriminating against or otherwise excluding individuals on the basis of race, color, or national origin in any of their activities.
A complaint must be filed no later than 180 days from the date of the alleged discrimination.
Should complete a SCS Complaint Form.
Should be forwarded as received to FNS Civil Rights Division or directly to an FNS Regional Office.
•Director will send a (20 day letter).
•Must be completed on the SCS Complaint Form.
•Verbal complaints should be written up by the person to whom the complaint is made.
•The complainant must provide the following information: name, address, phone number, location and name of the organization or office that is accused, name of the person complaint is against, details of the incident and the basis for the alleged discrimination (age, race, color, sex, disability, religious creed, national origin, or political belief).
•Complete a COMPLAINANT CONSENT/RELEASE FORM.
•Director will complete a SCS COMPLAINT ACCEPTANCE LETTER.
•Director will complete a REFERRAL LETTER.
•All information will be sent to FNS Civil Rights Division or directly to an FNS Regional Office.
